In order to assist you in the processing of information on your account, we have included forms to allow you to notify us of changes you wish to make. Please note that any change to the data you originally supplied us must be provided to us in writing.

Any changes to your account given us over the telephone or via e-Mail will be acknowledged but not executed until we have received the written and signed copy.

List of Available Forms

  • Contact List Change Form (pdf)
    • Please submit this form to change the names and numbers of the individuals who must be contacted by our Central Monitoring Station in the event of an incident. Please note that your contacts should know how your system works, and should have keys to the location.
  • Direct Deposit Payment Form (pdf)
    • Please submit this form to schedule recurring electronic funds transfers to remit payment on your account.
  • Credit Card Payment Form (pdf)
    • Complete and return this form to schedule payment on your account by credit card. Johns Brothers Security accepts Visa, Mastercard, Discover and American Express.
  • Change of Address Form (pdf)
    • Complete and return this form to notify us of any changes to your billing address, NOT the protected location.
  • Funding Request Form (pdf)
    • Use this form to request donations, funding or affiliation with Johns Brothers Security and its “Safe at Home” program.
  • IRS W-9 Form
  • Request for Service
    • Use this form to request any non-emergency, scheduled service to your installation.

City Registration Forms

Monitored systems in the Cities of Norfolk and Virginia Beach are required to be registered with the respective police departments. In Suffolk monitored and local systems which generate a signal visible or audible to others are required to be registered.
